An incident occurred while pulling tubing out of hole with a service rig. The snatch block failed and the sheave from the snatch block fell to the rig floor, landing 3-4 feet from the floorhand's position. The snatch block in use was rated for 1 tonne while previously an 8-tonne block had been used based on the calculated loads.
