Equipment Bucket Fell on Worker - Failure to follow Proper Procedure – Lost Time Incident

A team of workers was tasked with changing the bucket of an excavator. This excavator had hydraulic lines that required removal as well as the wedge in order to remove the bucket. While the bucket was curled inwards to expose the lines o a worker was unhooking the hydraulic lines of the bucket, o other workers removed the wedge, at the same time. The bucket rocked forward hitting the lone worker on the back of his head and left shoulder blade, knocking the worker down and forward.
