Summary

Incident:

Serious Incident Event 

Incident Description: 

A Driller hoisted the joint stopping part way to leave the casing joint pin end resting in the trough while several workers began installing the float collar onto  the stump. At this point, the catwalk operator, believing the second joint had been  fully picked up, began retracting the catwalk trough, inadvertently causing the pin  end of the casing to release from the trough and swing towards the rig floor  contacting a worker who was installing the float collar. The worker was struck on his hard hat/shoulder knocking him over. 

Learning From Past Incidents:

A worker sustained a minor soft tissue injury. 

We continue to receive safety notifications from companies that reference events from previous years. These submissions continue to help strengthen our understanding of incidents and how companies can improve processes and controls to keep workers safe.
